Social Worker for Geriatric Special Care Unit
The Geriatric Special Care Unit at Mitchell County Hospital Health Systems is looking for a full-time Social Worker. A $10,000 sign on bonus and excellent benefits are available with this position. Hours are Monday - Friday. The Special Care Unit Social Worker is responsible for coordinating and implementing a planned program of continuing care for the patient; functions on a multi disciplinary team and can act as a liaison among the health care team, and patients. Responsible for consultation and direction of social services in the unit, as well as provides orientation and education to the staff. Develops, organizes and implements programs, policies and procedures, conducts case work services and counseling and recommends functions necessary to ensure overall operational viability. Maintains performance improvement activities within the department and participates in CQI activities.

The Social Worker works under the direction of the Director of Clinical Services / Unit Director and works cooperatively with all personnel and ancillary staff.

Must possess a Master's degree in social work, licensing preferred. Must be credentialed or eligible for credentialing with privileges in social work. Must possess thorough knowledge of available area resource persons and agencies to facilitate supplemental treatment, transfer, and disposition of hospitalized patients.

Major Duties Include:
Conducts individual and group counseling sessions as primary therapist.
Conducts and records behavioral assessments of patients.
Conducts film or didactic presentations and subsequent discussion groups, relating the content to patient's life circumstances.
Charts observed patient (or family) progress on progress note sheets in the patient's record.
Participates as an active member of the interdisciplinary treatment planning team, assisting in defining goals and objectives and developing discharge plans for each patient.
Participates in discharge and aftercare planning for patients and aids patients in seeking therapeutic resources for follow-up in their own community environment.
Plans and arranges admissions to special care facilities as indicated by the individual patient needs, upon physician's request.
HIPAA: Authorized Access to Protected Health Information (PHI).
Employee performs duties pertaining to PHI usage as established (Minimum Data Set) according to expressed needs of employee when performing job duties as needed by policy and procedure and as defined by job description.
For CQI Audits:
*To review charts prior to being sent to Insurance companies, Lawyers, or other facilities.
*To compare present problems/conditions to previous ones when treating patients.
*Parts of the record usually needed but not limited to:
*Progress Notes, History, X-Ray Reports, Path Reports etc.
