Once a position has been offered, Applicants will be required to:
Successfully pass a drug screen, criminal background check, employment physical and TB test prior to beginning employment.
Provide documentation proving employment eligibility, such as a Passport, or a driver's license and Social Security card or Birth Certificate.
Provide immunization records, specifically MMR, Varicella and Hepatitis B
Provide bank account information for payroll direct deposit
Provide current professional licensure (if required)
Provide a copy of CPR certification (if applicable)
