Mission
The Health Information Management (HIM) Department is responsible for overseeing the timely processing, completeness and when necessary, the retrieval of all patient medical records.Services
A health record shall be maintained for every patient admitted for care in the hospital or who receives care as an outpatient.A patient or his/her personal representative may have access to his/her own health record upon presentation of a properly completed and signed authorization, according to the guidelines provided under the Health Insurance Portability and Accountability Act of 1996.
The hospital shall prepare a properly completed birth certificate on every birth and forward to the office of Vital Statistics. Click on the links for more information on HIPAA and for our Notice of Privacy Practice.
